Who chooses my physical therapist?

Your physician will most likely refer you to a physical therapist or offer a listing of physical therapists in the area. However, it is ultimately your choice which physical therapist you choose. Your insurance company may play a role in which you choose depending on coverage. Check our listing of accepted insurance companies to determine if your visit is covered by your insurance agency.

Our professional and knowledgeable staff combines 35 years of experience in physical therapy with a caring and compassionate touch. At Superior Rehabilitation, we aim to be your partner on the road to recovery, and strive to make your visits to our facility an encouraging, enjoyable experience.

 

What do I need to bring for my first visit?

On your first visit, please provide a valid ID, insurance card, completed patient forms, co-pay or co-insurance that may be due, a physician referral (unless self-paying), and your medication and medical history list. You can find the necessary patient forms here.

 

What should I wear?

Comfortable and casual attire is recommended for you physical therapy visits, as well as shoes that are appropriate for exercising. Also, please bring shorts to your first visit if you have a lower back or leg diagnosis.

 

How do I make an appointment?

Our office hours are 7:30 a.m. to 6:00 p.m. Please contact our office at (504) 887-7463 to make an appointment.
